As a self-employed dentist, doctor, pharmacist or locum, your tax position is more complex than most. Here's what you need to know.

NHS vs Private Split

If you work across both NHS and private practice, your income needs to be carefully split for tax purposes. You can claim equipment, lab fees, and materials against each income stream.

Going Self-Employed? Key Dates

Planning to go self-employed as an associate from September 2026? You'll need to register for Self Assessment by 5 October 2027.
Your first tax return, balancing payment and first payment on account are then due 31 January 2028.

Why Your First Tax Bill Is Bigger

Your first bill isn't just what you owe for the year. HMRC also adds an advance "payment on account" toward next year's tax. A £1,800 tax bill can mean paying £3,600 in January, with a further £1,800 due the following July.

Start Saving From Day One

Putting aside 25–30% of your income into a dedicated savings account from your very first pay cheque means the funds are there when that bigger bill arrives.

Dentist & Associate Expenses

Claim GDC fees, indemnity insurance, BDA/DPA membership, lab fees you personally cover, equipment like loupes, CPD courses, mileage between practices, and accountancy fees.

NHS Pension Considerations

NHS pension contributions are one of the most valuable tax reliefs available. Higher-rate taxpayers receive 40% relief on contributions. It's important to declare these correctly on your Self Assessment.

Should You Incorporate?

Once your profits exceed a certain threshold, operating through a limited company can be more tax-efficient. We'll model both scenarios so you can make an informed decision.

Making Tax Digital for Self Assessment

From April 2026, self-employed dentists and associates earning over £50,000 must keep digital records and file quarterly updates through MTD-compatible software, with the threshold dropping to £30,000 from April 2027. We'll get your records MTD-ready well ahead of your mandation date.

Starting Mid-Year? MTD May Not Apply Yet

Start partway through the tax year, say in August, and your pro-rated income could fall under the MTD threshold even if your full-year equivalent wouldn't. From April 2028 though, the threshold drops to £20,000, bringing most associates into scope.

Locum vs Employee Tax

Whether you're a locum pharmacist or employed, your tax obligations differ significantly. Locums must register for Self Assessment and manage their own National Insurance.

Claimable Expenses

Pharmacists can claim: GPhC registration, CPD, professional indemnity, uniform costs, travel between pharmacies, and equipment or reference materials.

Multiple Income Sources

Many pharmacists have income from multiple locum agencies or employers. We ensure all sources are declared correctly and that you're not paying more tax than you owe.

Pension Planning

Self-employed pharmacists miss out on employer pension contributions. We help structure personal pension payments to maximise tax relief and plan for retirement efficiently.

Making Tax Digital for Self Assessment

If your self-employed or locum pharmacy income is above £50,000, MTD for Income Tax now requires digital record-keeping and quarterly submissions rather than one annual return, falling to £30,000 from April 2027. We'll set you up on compliant software before it becomes mandatory.

GP Partners & Salaried GPs

GP partners have partnership income with specific allowable expenses. Salaried GPs may also have private income to declare. Both require specialist handling to ensure accurate returns.

Splitting NHS & Private Income

Doctors with both NHS and private income sometimes don't split it correctly, meaning private earnings end up under-declared or over-taxed. Each income stream needs to be tracked and reported separately, with its own allowable expenses.

NHS Pension & Annual Allowance

The NHS pension scheme is generous but complex. Doctors approaching the annual allowance limit face significant charges. We help you understand your position before it becomes a problem.

Personal Service Companies

Many doctors use limited companies for private or locum work. We advise on IR35 status, director salary and dividend structure, and ensure HMRC compliance.

Making Tax Digital for Self Assessment

GP partners and doctors with private or locum income over £50,000 now fall under MTD for Income Tax, meaning digital records and quarterly updates instead of a single annual return, with the threshold falling to £30,000 from April 2027. We handle the transition so nothing slips through the cracks.

Registering as Self-Employed

All locums must register with HMRC for Self Assessment. Failing to register or filing late results in penalties. We handle your registration and ensure deadlines are never missed.

Travel & Mileage Claims

Locums travel extensively between sites. HMRC allows 55p per mile for the first 10,000 miles (increased from 45p in April 2026). Keeping accurate mileage logs can add up to a significant tax saving.

Expenses You Can Claim

Locums can claim: professional registration, indemnity insurance, CPD, medical equipment, stationery, and a proportion of phone and home office costs.

Payments on Account

HMRC requires locums to make advance tax payments (Payments on Account) twice a year. Many are caught off-guard by these. We forecast your liability so you're always prepared.

Expert advice for landlords

Property tax rules change often, and it's easy for something to slip through. Here's what every landlord should know.

Rent a Room Scheme

Letting a furnished room in your own home comes with a separate tax-free threshold under the Rent a Room scheme. Many landlords don't realise it applies, or declare the income incorrectly against their main rental property instead.

Mortgage Interest Relief

Mortgage interest can no longer be deducted in full from rental income. It's given as a 20% tax credit instead, and landlords who still calculate it the old way often end up with the wrong tax bill.

Property Income Allowance

If your rental income is modest, you may be better off claiming the £1,000 property income allowance instead of itemising expenses. Many landlords aren't aware this option exists.

Replacing Domestic Items

You can claim the cost of replacing furniture, appliances and furnishings in a let property, but not the cost of furnishing it for the first time. The distinction catches a lot of landlords out.
